Electronic Liability Waiver

Athletics Release

As a parent/guardian of a participant of the Real Betis Texas Summer camps, I realize that inherent dangers exist. While my son/daughter is in good health and able to fully participate in such activities, I realize their participation may result in illness or injury due to accidents, the forces of nature or other unforeseen causes. Such illness or injury may include disease, strains, sprains, fractures, dislocations, paralysis and/or death. By participating, I hereby and knowingly assume all risk resulting from these activities.

On behalf of myself, my family or other personal representatives I hereby agree to release, hold harmless and indemnify Global Sports Development Group, LLC, its agents, officers and wilderness leaders from any and all claims and lawsuits for doily injury, property damage, loss of services etc. which may result from my participation in the above mentioned activities, regardless of weather or not these claims or suits arise from participant, any other person involved or from any other cause.

Financial Policies & Refunds

• Payment plans are not offered.
• Refunds are not available and will not be provided.
• If you cannot attend the camp after you have registered, credit can be provided for a future camp in the same city or state the following year or edition.

Use of Materials for Promotional Purposes

I/We hereby give permission to Real Betis Texas Summer Camps & Global Sports Development Group, LLC to use pictures and videos of the players to promote events organized by the organization.

Acknowledgment

I HAVE READ THE ABOVE WAIVER AND RELEASE I UNDERSTAND THAT I HAVE RELINQUISHED SUBSTANTIAL RIGHTS BY SIGNING IT AND DO SO VOLUNTARILY.
